Consent Form/誓 約 書

・I am not, and do not have any relationship with, a member of gangster organization stipulated in Organized Crime Exclusion Ordinances.

・I am over 18 years old and am not currently in high school.

・I am not misrepresenting my identity or age. If I receive a treatment through the misrepresentation of my identity or age, the artist will not be liable for any damages caused by the treatment and I acknowledge that I will compensate for all the damages incurred by the artist.

・I am not in a physical condition being unfit to undergo treatment due to illness, drug use, or drinking, etc. If the artist decides that it is impossible to perform the treatment, I agree to follow the artist's judgment.

・I understand that the ink used in tattoos may cause allergic reactions.

・I understand that tattoos will change with the passage of time, and will not object to the tattoo that I have undergone and completed no matter what changes occur after completion.

・I understand that the tattoo is permanent which can be removed only by medical operation.

・I have confirmed that the needles and tubes used for the treatment are new or sterilized.

・I do not use the design created by the artist for commercial purposes without the permission of the artist.

・I have checked my tattoo design created by the artist before the treatment.

・I agree that the artist will post a photo of my tattoo that he/she has designed or operated on the Internet or in print, whether before or after the treatment is completed.

・私は暴⼒団排除条例に定める暴⼒団員等ではありません。

・私は、⾝分や年齢を詐称していません。私が⾝分や年齢を詐称して施術を受けた場合、アーティストは⼀切その責任を負わず、私はアーティストが被った損害すべてを補償することを了承します。

・私は体調不良、薬物使⽤、飲酒等によって施術を受けるに相応しくない状態ではありません、また、アーティストが施術不可能と判断した場合には、アーティストの判断に従うことを了承します。

・私は、タトゥー施術に使⽤されるインクによってアレルギー反応が起きる可能性があることを了承します。

・私はタトゥーが永久的なものであることを了承し、⼿術によってのみ除去が可能であることを理解しています。

・私は、タトゥーが時間の経過と共に変化をすることを理解し、私が施術を受けて完成したタトゥーについて、完成後いかなる変化が⽣じても異議申し⽴てをしません。

・私は、アーティストが私に施術するタトゥーのデザインを事前に確認しました。

・私は、施術に使われる針やチューブ等が新品⼜は滅菌済みであることを確認しました。

・私は、アーティストの作成したデザインを、アーティストの許可なく営利⽬的で使⽤しません。

・私は、アーティストが当該デザインまたは施術したタトゥーの写真を、施術の完了前後を問わず、インターネットまたは印刷物などの媒体に掲載することを了承します
